Enrollment Instructions

Thank you for your interest in NYU OnsiteHealth for Physicians.

In order to gain access to NYU OnsiteHealth you must complete two simple steps outlined below. After you have completed these steps your request will be sent to the Information Technology Department for processing. Once your request has been processed you will receive an e-mail regarding your request status and information for logging on for the first time including instructions, your user name and password.

1. Read the System Requirements to ensure you are aware of the appropriate hardware needed to support NYU OnsiteHealth. Select "Continue".
2. Fill out the enrollment form and submit. Select "Submit this form". You will receive a confirmation number after submission.  You will receive a confirmation number after submission. Please keep this number for future reference.
   
   Please be sure to read the terms & conditions (this information addresses confidentiality, security and e-mail) and select "Accept" if you agree. By selecting accept, you are agreeing to the terms & conditions for using NYU OnsiteHealth.

The Information Technology Department will process your enrollment request. You will receive an e-mail with your request status. If you are approved for access you will receive a user name and indication of your password, as well as detailed instructions to assist you in logging into NYU OnsiteHealth.
